ISA Survey Participation Form

Thank you for considering volunteering to take the ISA Survey aimed at establishing the relevance of the Jobs Tasks that will be used in the development of the Certified Automation Professional (CAP) test. After you submit this completed form, there will be a delay of a few weeks before you will receive the actual survey from ISA. If ISA receives more than 1500 requests, they will filter some, so if you don't receive the survey it is because they had more than 1500 requests.
